
Community Health Workers
CareNet employs community health workers (CHWs), who work one-on-one with low-income clients to get connected to medical care and social services. CHWs assist clients who are pregnant, of childbearing age, adults with chronic conditions, and other vulnerable populations.
CareNet CHWs can help residents with:
- Access to transportation
- Access to specialty care
- Access to legal services
- Access to smoking cessation
- Access to food/clothing/housing
- Having a healthy baby
- Reducing medically unnecessary emergency department visits
- Addressing medication non-compliance
- Addressing missed medical appointments
- Referrals to other community agencies
Medical professionals with non-pregnant patients who are on Medicaid or uninsured and could benefit from one-on-one assistance from a CHW should complete the community health worker referral form. CareNet’s CHWs work through the Northwest Ohio Pathways HUB, a community-wide care coordination system.
