Membership Requirements
CareNet is not health coverage or medical insurance. Instead, CareNet is a non-profit partnership that operates a charity care network. This network helps qualifying low-income Lucas County residents get medical care and services for free or at reduced cost.
To enroll in CareNet’s charity care network, members must be Lucas County residents for at least six months and cooperate with the application process. Additionally, CareNet members must be ineligible for government healthcare assistance and have no access to affordable private coverage. They also must have a household income up to 200% of the Federal Poverty Guidelines, which are listed at right for 2025.

- Household of 1 up to $31,300
- Household of 2 up to $42,300
- Household of 3 up to $53,300
- Household of 4 up to $64,300
- Household of 5 up to $75,300

Enrollment
For Lucas County residents who already have family physicians, please contact their office to see if they accept CareNet. If they do, please contact CareNet at 419-820-0161 for instructions on how to complete the process.
If you do not have a family physician, or your family physician is unable to see you as a CareNet member, please choose a primary care site from the application at right that best meets your needs. Call that clinic and ask to schedule a new patient appointment. You will need to take a completed application along with a photo ID and income verification with you to your new patient appointment.
